The primary responsibility of your job position is to provide Social Work Consultation services to communities and identified consulting contracts and special projects.  Provides information and makes recommendations related to resident care psychosocial concerns, regulatory compliance review/guidance, mentoring, providing education, and facility practices following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities.   

KEY RESPONSIBILITIES:

Community Staff Engagement               40% % time

  • Provides social work consultation services on a monthly basis (unless otherwise contracted) to each of community assigned
  • May assist/participate in learning circles to address departmental concerns with community staff
  • Develop familiarity with community visioning plans in addition to partnering with community and culture change team to assist and provide support with identified goals and implementation
  • Provide annual survey support to communities as needed/requested
  • Provide annual survey support to communities as needed/requested
  • Attend and participate in Operations meetings
  • Attend, coordinate, and participate in psychosocial clusters
  • Participate in Company or community committees (e.g. Ethics Committee)
  • Schedule and conduct monthly behavior case review meetings with community social service staff
  • Attend monthly culture change collation meetings
  • Will be available to community staff to assist in event of emergent situations regarding residents needs/concerns
  • Some overnight stays may be required

Training and Guidance:              40% time

  • Provide systems training/guidance to new social services staff
  • Pursue ongoing Eden/culture change educational opportunities
  • Participate in baseline assessments and pre-surveys with the Quality Assurance Team as schedule requires
  • Present training related to psychosocial issues/challenges upon identification or request, during scheduled consultation visits
  • Participate in approved workshops and educational sessions
  • Develop on-going educational and experiential opportunities for all community social service staff in addition to MSW graduate student field placements

 

Administrative Support Duties:              20 %time

  • Provide consultation with nursing home administrator and social service staff to include review of selective resident records as identified by staff to ensure compliance with state and federal regulations in addition to company policy and procedures
  • Accurate accounting of expenses and consultation time within required time frames
  • Work on developing policies and procedures for social services and related areas
  • Perform all other duties, as assigned

 

SKILLS:

  • Must possess a current, unencumbered, active license to practice in the state
  • Must have excellent documentation skills
  • Must have basic computer skills

 

Knowledge:  

  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities



EDUCATION AND EXPERIENCE:

Education: Must possess, as a minimum, a Bachelors of Social Work Degree from an accredited college or university and Colorado Licensed Clinical Social Worker. Licensed Clinical Worker with Long Term Care experience and working on completion of clinical hours may be considered.

 

Experience: Must have, as a minimum, two (2) years of experience as a social worker in Long Term Care.  Must have experience working with the psychiatric population. Experience in healthcare consultant role preferred

 

PHYSICAL REQUIREMENTS:

  • Must meet the general health requirements set forth by the policies of this facility, which include a mental and physical examination
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility
  • Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met
  • Must be able to push, pull, move, and /or lift a minimum of 25 pounds to a minimum height of 5 feet
  • Be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet
  • May be necessary to assist in the evacuation of residents during emergency situations
